Yorkshire Ambulance Service has launched an investigation into two staff members after claims they may have accessed or tried to access medical records without legitimate permission. The trust responded to emergency calls on Saturday, when William Chapman climbed into a tiger enclosure at Yorkshire Wildlife Park near Doncaster and was killed.
